Your NPI number is the only number you'll ever need for electronic transactions. U.S. regulations require you to submit HIPAA-standard transactions with this number.

It's easy to apply for your NPI

You can choose one of three ways to apply:

Complete the application on the website of the Centers for Medicare and Medicaid Services.
Apply for an NPI online
Call 1-800-465-3203 to request a paper application.
Give your permission to an organization, such as a group practice, that will apply for you.

Share your NPI with us

You can send us your NPI using any one of these options:

Fill out our online form
Call our National Dentist Line: Phone 1-800-451-7715 to submit your NPI to us.
Send a fax: Fax your NPI information to 1-859-455-8650.
